Coronary heart disease - a fairly common disease state, which is accompanied by relative or absolute violation of myocardial perfusion as a result of heart coronary arteries impairment. In a more accessible language, the coronary heart disease - lack of oxygen to the myocardium. This disease can make itself felt in the form of myocardial infarction or angina attack. Another form of chronic ischemic heart disease is ischemic cardiomyopathy.
Therapy of dilated cardiomyopathy is a treatment to prevent complications of heart failure. With the use of ACE inhibitors, which reduce blood pressure. Currently, the most popular in this case is Coreg.
Coreg or generic Carvedilol - alpha-beta-blocker, is one of the most successful drugs in the treatment of heart disease, since it has a positive antioxidant effect on the myocardium. The effect of this drug is detected in the control of body weight and blood electrolyte composition. These parameters should be monitored several times during the week.
Indications for Coreg use:
• hypertension (monotherapy or in combination with other antihypertensive drugs)
• angina (stable)
• chronic cardiovascular failure.
Are there any contraindications of the drug Coreg?
Be attentive when take Coreg and count all prescribed contraindications:
- hypersensitivity to Carvedilol or any of the excipients of the drug;
- decompensated heart failure, COPD clinical liver dysfunction, BA; AV-block II and III degree, bradycardia (<50 bpm. / Min);
- cardiogenic shock;
- sick sinus syndrome (including sinus block of the heart);
- arterial hypotension;
- metabolic acidosis;
- Prinzmetal angina;
- severe violations of peripheral arterial circulation;
- joint therapy with verapamil or diltiazem;
- pregnancy and lactation.
How best to use Coreg?
Carvedilol is taken by mouth, regardless of the meal. If a patient suffers cardiovascular failure, it is recommended to use the drug with meals (for an increase of absorption, which reduces the risk of orthostatic hypotension).
When hypertension, the recommended mode of Coreg application - 1-2 times a day. Adult initial dose - 12.5mg/day first 1-2 days. The maintenance dose is 25mg/day.
Patients with stable angina - the first 1-2 days 25mg/day, divided into 2 doses. Maintenance Coreg dosage - 50mg/day. The maximum recommended dose - 100mg/day (2 doses).
Carvedilol is appointed as an adjunct to conventional treatment with angiotensin-converting factor, diuretics, digitalis drugs group and vasodilators.
There may be some worsening of symptoms of cardiovascular disease at the beginning of treatment (especially in patients who use high-dose diuretics and (or) with severe cardiovascular disease). In these cases, removal of Coreg is not required, it is only necessary to refrain from increasing the dose.
While treatment with Coreg, the general condition of patient should be monitored by cardiologist.
What side effects may occur with Coreg treatment therapy?
If you take the medication according to prescription, there should not be any side effects, but you must know them in order to be in case: thrombocytopenia (mild), peripheral edema, hyperglycemia, sleep disorders, headache, depression, dizziness, visual impairment, renal failure, diarrhea, vomiting, impotence, pain in the extremities, dyspnea, psoriasis, swelling at the injection site, weakness. Rare side effects: angina pectoris, atrioventricular block, exacerbation of symptoms of peripheral vascular disease (Raynaud's syndrome, intermittent claudication, etc.).
Intake of Coreg may provoke latent diabetes mellitus, worsening of diabetes, poor control over the content of serum glucose levels.
