Trileptal in the treatment of epilepsy
One of the latest AEDs, synthesized in recent years, is oxcarbazepine (Trileptal), which is known by its potent anticonvulsant effect.
Currently, a large number of worldwide studies had proven clinical efficacy and relative safety of Trileptal in the treatment of epilepsy. A review article by Schmidt & Elger reported 21 publications on the use of Trileptal in 2191 epileptic patients. In general, it shows high efficacy and good tolerability.
Medical specialists consider this remedy a basic drug for epilepsy in children, adolescents and adults.
Trileptal is recommended to be included as a starting monotherapy of epilepsy with simple, complex and secondarily partial generalized seizures.
What are the medical indications for Trileptal drug?
First of all, Trileptal is taken in monotherapy (as first choice), as well as in combination therapy: simple and complex partial seizures (with or no loss of consciousness), including secondary generalization and generalized tonic-clonic seizures.
The drug has also some contraindications, which should be taken into account before therapy: hypersensitivity, pregnancy and lactation. The use of the drug in children, younger than 2 years, has not been studied.
How to pick up the correct Trileptal dose?
The remedy is prescribed inside, regardless of the meal, with a small amount of liquid. You can easy take suspension directly from the syringe or dilute it in some water. In monotherapy or in combination with other anti-epileptic drugs, Trileptal dose should be divided into 2 doses. The dose may be increased depending on the response to therapy.
Monotherapy with Trileptal starts with 600 mg / day (8-10 mg / kg / day ), divided into 2 doses. The dosage may be gradually increased at weekly intervals of not more than 600 mg / day to achieve the desired therapeutic response.
The recommended starting dose for children older than 2 years - 8-10 mg / kg body weight per day, divided into 2 doses both in monotherapy and in combination therapy.
Trileptal clinical research
A lot of early and recent medical works have proved high efficacy and tolerability of Trileptal in adults and children older 2 year.
This study compared the efficacy of three doses of oxcarbazepine therapy (600 mg, 1200 mg and 2400 mg daily) with placebo in the mode of combination therapy in 694 patients, aged 15 to 65 years. Frequency 50 % response rate was 12.7 % in the placebo group, compared with 26.8% of patients receiving oxcarbazepine 600 mg per day, 41.2% of patients receiving 1200 mg per day, and 50% of patients receiving 2400 mg per day. Mean reduction of seizure frequency was 6.8 %, 22 %, 40 % and 50 % respectively.
